Quarterly Governmental Report

Sort Options

Select an option for sorting the report.

 

Form Code

Enter a code to identify the format to use to print governmental reports, or click the Lookup button to list all report form codes. The default code of 1 represents the 941A Format. After the form code is entered, enter a description. A separate form code can be established for different report formats. To define a new report format, enter a new form code and a description. Click Form to access the Forms Customization window.

 

Optical Scan

Select this check box to remove all commas from amounts and name fields, and dashes from Social Security number fields. Clear this check box if you do not want to print the report in an optical scanner format.

 

Print Federal Data

Select this check box to include employee quarter-to-date wages and federal taxes. Clear this check box to exclude federal data from the report.

 

Print State Data

Select this check box to include employee quarter-to-date wages and state taxes for the state code(s) specified in the State Code field. Clear this check box to exclude state data from the report.

 

Print Local Data

Select this check box to include employee quarter-to-date wages and local taxes for the local tax code(s) specified in the Local Code field. Clear this check box to exclude local data from the report.

 

State Code

If you selected the Print State Data check box, enter a state code, or select the All check box to print state tax information for all states. Click the Lookup button to list all state codes.

 

Local Code

If you selected the Print Local Data check box, enter a local code, or select the All check box to print local tax information for all local areas from one state (you must first select a state in the State Code field). Click the Lookup button to list all local codes for the state selected in the State Code field.

 

Employee Number

Enter a range of employee numbers, or select the All check box to include all employees. Click the Lookup button to list all employee numbers.

 

Sort Field

Enter a range of sort codes, or select the All check box to include all sort codes.

 

Employee Name

Enter a range of employee names, or select the All check box to include all employees.

 

Social Security Number

Enter a range of Social Security numbers, or select the All check box to include all Social Security numbers.
