Select Library Master Setup menu > Report Manager. In the Report Manager window, click <create new form> from the list of existing Crystal forms.

Overview

Use the <create new form> option to define a new faxable form. Faxable forms can be customized by user ID and/or company code. Click Accept to select the Crystal template from which to base the new faxable form.
