Covered Individuals - Fields

 

Covered Individuals

Calendar Year

This field displays the calendar year for which you are entering information and can only be viewed.

 

Employee No.

This field displays the selected employee's number and can only be viewed.

 

Name

This field displays the selected employee's name and can only be viewed.

 

Covered Individual Name

Enter the name of the covered individual. When entering the employee's name, it should exactly match what is displayed in the Name field to avoid reporting errors.

 

Social Security No.

Enter the covered individual's Social Security number. If the individual does not have a Social Security number, enter his or her date of birth in the Date of Birth field.

When entering the employee's Social Security number, it should match the one entered in Employee Maintenance to avoid reporting errors.

 

Date of Birth

If the covered individual does not have a Social Security number, enter his or her date of birth. Click the Calendar button to select a date from the calendar.

 

Covered All Months

Select this check box if the individual was covered for at least one day in every month of the year. Clear this check box if the individual was not covered for at least one day in every month.  

 

Months

If the individual was not covered for at least one day in every month of the year, select the check box for each month in which he or she was covered. Clear the check box for any month for which the individual was not covered, or clear all check boxes if the Covered All Months check box is selected for the individual.
